COPD Outreach Service

Service Description

Chronic Obstructive Pulmonary Disease (COPD) Outreach provides a 'Hospital at Home' (HaH) service for patients who fit the criteria and who present to the Emergency Department with an exacerbation of COPD.  The service was set up under the guidance of Dr. David Curran.  This service is offered to patients who live within a half-hour (30 minutes) drive of Mercy University Hospital.

Up to 4 home visits are performed over a two-week period.

Patients are then discharged back to the care of their General Practitioner (GP).

Team

The team includes Dr. David Curran - Consultant Respiratory Physician, Ciara Hanrahan - Senior Physiotherapist and Jennifer McKiernan, Respiratory Clinical Nurse Specialist.  

The COPD Outreach team works in conjunction with the respiratory team, patients own GP, public health nurse and community allied health professionals (for example:  physiotherapists, pharmacists, etc.).
